CSG New Hire Setup Form

CSG has updated its new hire setup form to make it easier to fill out and submit. This short how-to explains how to fill it out. You can download the form here by clicking the link below:

 Instructions

  1. The form is a simple Excel sheet with fillable fields. Start by entering the new hire’s name, start date, job title, business unit, supervisor, building and office/room. The building and office/room locations will determine where computer equipment, if any, will be set up.

  2. Enter the new hire’s Kerberos ID. This is a mandatory field and the form will not be completed without it.

  3. Select the employee type. This is a drop-down field with 3 choices. If the hire is a temporary employee, you will be prompted to enter an end date. If the hire is a student, you’ll be prompted to enter whether or not they need a work (OU) email address.

  4. Select whether the new hire was previously a UC Davis student using the drop-down list.

  5. Enter any relevant notes in the Notes field. Let us know of any mailboxes, software, or special equipment the new hire may need.

  6. Finally, if the new hire will need access to StarRez, that must be requested in a separate form. Click the link to go to the IT Knowledge Base where you can download the StarRez access request form. For StarRez access, a completed FERPA certificate is required. See the page: How to Request Access to StarRez.

  7. Once your form is complete, save it as a standard Excel file and attach it to a service request and send to the SHDS Helpdesk.
