To track change requests we use an customized Approvals template in Teams. Here is how to create and fill out a change request using this template.

Create a step-by-step guide:

  1. First, navigate in Teams to the Change Requests Channel for the SHDS | Computer Services Group team.

  2. Once there, click on Start a post.

  3. Then click on the '+' sign and then Approvals

  4. Click on the Change Request template

  5. Give the change request a title, add the approvers and include a description. Do not include yourself as one of the approvers.

  6. Scroll down to continue to fill out the form, providing the urgency level, risk level, upgrade and rollback plans

  7. Last, provide the requested time window and add the start time in the notes along with any other useful notes

  8. Click on the Send button to Post your Change Request

  9. Once all approvers have signed off on the change request, you may then proceed.

While the note about approvers says that Jerry, Martin, Eddie and Andrew are the standard approvers, I recommend adding Chris, Eddie, and Andrew along with Jerry and Martin.
